Annex 1 Mandatory Conditions
1. No supply of alcohol may be made under this premises licence
(a) at a time when there is no designated premises supervisor in respect of the premises licence, or
(b) at a time when the designated premises supervisor does not hold a personal licence or his personal licence is suspended.
2. Every supply of alcohol under this premises licence must be made or authorised by a person who holds a personal licence.
Annex 2 Converted Conditions from a Justice on Licence
1. No intoxicating liquor shall be supplied otherwise than to a person who has attained the age of 18 years and is:
1.1 a member of the club who has been a member for at least 2 days or whose application or nomination for membership was made at least 2 days before his admission to the club
1.2 a bona-fide guest on such a member
2. Persons attending the premises of the said club as players, officials or spectators in connection with any sporting event taking place at the club’s premises.
3. Persons attending at the said club in connection with any pre-arranged bona-fide private functions, conferences or meetings taking place there provided that no notice of such functions excluding conferences and meetings is given to the chief constable 48 hours prior to the holding of such a function such notice to contain the name and address of the person or organisation organising the function, the number of people attending and the purpose of the function.
4. There shall be no off sales of intoxicating liquor except a member in person.
5. There shall be no application for any occasional licences.
6. That a copy of the rules of the club shall be posted to the Licensing Department and the South Yorkshire Police Constabulary
7. That any change in any of the club rules shall be notified within 14 days of such a change to the Licensing Department and to the South Yorkshire Police Constabulary.
8. There shall be no alterations to the rules relating to the admission of guests to the club without the consent of the Licensing Authority.
9. That a list of members’ names and addresses be kept on the premises and shall be provided to a Police Constable in uniform on request.
10. A copy of the club rules must be given to every member upon his election to the club.
11. There shall be no application for a special hours certificate.
12. Any sign or advertisement displayed in public shall indicate a reference to a member’s club.
13. Members shall be permitted to introduce guests but no member may introduce more than 3 quests at any one time.
