Annex 1 Mandatory Conditions
Mandatory conditions where licence authorises supply of alcohol
1 No supply of alcohol may be made under the premises licence:-
a at a time when there is no designated premises supervisor in respect of the premises licence,
or
b at a time when the designated premises supervisor does not hold a personal licence or his personal licence is suspended.
2 Every supply of alcohol under the premises licence must be made or authorised by a person who holds a personal licence.
Annex 2 Conditions consistent with Operating Schedule
General
1. The licence holder shall ensure staff training takes place every quarter or on evey occation that a new member of staff is employed.
Prevention of Crime and Disorder
2. There shall be a CCTV system set up to the minimum approved standards as set by South Yorkshire Police.
3. No person under the age of 18 years shall be permitted on the premises at such times the sale of alcohol takes place.
4. A maximum capasity figure shall be set in accordence with a Fire Risk Assessment undertaken on the premsies at each time the premises is open to the public.
5. There shall be a minimum of 2 SIA registered door staff, at least 1 of which must be a female, in attendence at all such times as the premises is open to the public.
6. The Designated Premises Supervisor and Premises Licence Holder must adhere to the South Yorkshire Police "Drugs & Weapons Policy".
7. The licence holder & Designated Premises Supervisor shall maintain membership of the Licence Watch scheme.
8. A Personal Licence Holder shall be on the premises at all times the premsies is open to the public.
9. The premises shall at all times adhere to and operate the "Challenge 21" scheme.
10. That no drink or bottles are alowed to be taken from the premises.
11. On any night the premises is operating a "Basline" event or dance events with multiple DJ's playing (excluding a premisis licence holder) the following conditions will apply:
i) Every customer will be searched on entry to the premises.
ii) Every customer will be required to supply photographic ID and that ID will be scanned using a South Yorkshire Police approved ID scanning device.
12. On any night other than in 10. above the following conditions will apply:
i) Every customer will be searched on entry to the premises at the discression of SIA registered door staff or management of the premises.
ii) At the discression of SIA registered door staff or management of the premises a customer will be required to supply photographic ID and that ID will be scanned using a South Yorkshire Police approved ID scanning device.
13. No glasses or bottles shall be issued to any customers; excepting Champagne bottles which shall be supervised by a member of staff and removed immediatly on emptying from the customers.
14. The premises shall operate the required "ID Scan" or other suitable ID scanning device in consultation with South Yorkshire Police and may only cease to use any approved ID scanning device upon prior written agreement with South Yorkshire Police.
15. The premises will not operate any "once entry price paid drinks free" or any "all inclusive" promotions of any type. Should any agreements be reached with any Pub/Club or Licence Watch schemes reguarding minimum price charge agreements or similar schemes the premises licence holder/DPS will agree to sign as a member of this scheme.
Public Safety
16. The licence holder shall ensure regular fire and safety checks are undertaken on the premises.
Prevention of Public Nuisance
17. The licence holder shall ensure that notices are displayed in promenent places about the premises asking customers to be quiet when leaving the premises.
Protection of Children from Harm
18. The licence holder shall ensure "blanks" are places in any unused electircal sockets.
15. The licence holder shall ensure that children unable to come in to contact with any type of heat source.
