Annex 1 - Mandatory Conditions
1 No supply of alcohol may be made under the premises licence
a) At a time when there is no designated premises supervisor in respect of the premises licence, or
b) At a time when the designated premises supervisor does not hold a personal licence or his personal licence is suspended.
2 Every supply of alcohol under the premises licence must be made or authorised by a person who holds a personal licence.
3 The premises licence holder shall ensure that an age verification policy applies to the premises in relation to the sale or supply of alcohol.
The policy must require individuals who appear to the responsible person to be under 18 years of age (or such older age as may be specified in the policy) to produce on request, before being served alcohol, identification bearing their photograph, date of birth and a holographic mark.
Annex 2 - Conditions consistent with Operating Schedule
Prevention of Crime and Disorder
1 The licence holder shall ensure that sales of alcohol for delivery will only be made with a food order including a main meal.
2 The licence holder shall ensure that consumption of alcohol does not take place within the customer car park.
3 The licence holder shall ensure that that all staff involved in the sale of alcohol receive training on customer relations, and in particular how to deal with abusive/violent customers.
4 The licence holder shall ensure that they are a member of Rotherham Licence Watch Scheme.
5 The licence holder shall ensure that all deliveries of orders containing alcohol are made from within a delivery van which has been specially adapted with a metal grill where food and alcohol will be dispensed to customers.
6 The licence holder shall ensure that a CCTV camera is fitted to the delivery vehicle to capture the image of persons taking delivery of alcoholic products.
7 The licence holder shall ensure that CCTV is installed at the front and rear of the premises and is maintained and in working order at all times.
Public Safety
8 The licence holder shall ensure that staff have adequate training (including first aid) to secure the safety of the premises and its patrons.
9 The licence holder shall ensure that there is regular testing and certification of fire fighting equipment and systems pertinent to safety.
Prevention of Public Nuisance
10 The licence holder shall ensure that regular checks are made to remove any litter in the immediate vicinity of the premises.
11 The licence holder shall ensure that all customers respect the need to arrive and leave the premises quietly.
Protection of Children from Harm
12 The premises shall operate in accordance with "Challenge 21" and all customers attemption to purchase alcohol who appear to be under the age of 21 shall be required to produce a valid proof of age. Notice to this effect shall be displayed in a clear and prominent position at the premises.
13 The only proof of age accepted in respect of the sale of alcohol shall be: a proof of age card bearing the "PASS" hologram logo; a passport; an EU photo driving licence; or any other form of identificationapproved in writing by the Licensing Authority.
14 A refusals book, or similar record, shall be kept at the premises in which must be recorded forthwith the date, time and circumstances under which any attempted purchase by a young customer has been refused. This book, or other form of record, must be avalible for inspection, upon demand, by any police officer, or authorised person.
