Annex 1 Mandatory Conditions
Mandatory conditions where licence authorises supply of alcohol
1 No supply of alcohol may be made under the premises licence:-
a at a time when there is no designated premises supervisor in respect of the premises licence,
or
b at a time when the designated premises supervisor does not hold a personal licence or his personal licence is suspended.
2. The premises licence holder shall ensure that an age verification policy applies to the premises in relation to the sale or supply of alcohol.
The policy must require individuals who appear to the responsible person to be under 18 years of age (or such older age as may be specified in the policy) to produce on request, before being served alcohol, identification bearing their photograph, date of birth and a holographic mark.
Annex 2 Conditions consistent with operating schedule
Prevention of Crime and Disorder
1. The licence holder shall ensure that CCTV is installed and operated at the premises. The images recorded by the system shall be retained in an unedited format for a period of not less than 31 days and made available to an officer of a responsible authority upon request.
Public Safety
2. The licence holder shall ensure that a first aid box is kept at the premises.
3. The licence holder shall ensure staff are trained on the premises’ evacuation procedure.
Prevention of Public Nuisance
4. The licence holder shall ensure that the external perimeter of the premises is kept clean and tidy.
Protection of Children from Harm
5. The licence holder shall operate the ‘challenge 25’ proof of age scheme and all staff trained in its implementation.
6. The licence holder shall ensure that only passports, photo card driving licences and those featuring the PASS logo will be accepted as valid forms of identification.
7. The licence holder will ensure that signs are located at the point of sale and on the shelves where alcohol is kept advising customers that the Challenge 25 policy is in force.
8. The licence holder shall ensure that staff are trained in the law regarding underage sales, and training will be repeated at regular intervals.
9. The licence holder shall ensure that a refusals log is kept at the premises and made available for inspection to an officer of a responsible authority upon request.
10. The licence holder shall ensure each till incorporates a prompt system in relation to age restricted items.
