Annex 1 Mandatory Conditions
Mandatory conditions where licence authorises supply of alcohol
1 No supply of alcohol may be made under the premises licence:-
a at a time when there is no designated premises supervisor in respect of the premises licence,
or
b at a time when the designated premises supervisor does not hold a personal licence or his personal licence is suspended.
2 Every supply of alcohol under the premises licence must be made or authorised by a person who holds a personal licence.
3 The premises licence holder shall ensure that an age verification policy applies to the premises in relation to the sale or supply of alcohol.
The policy must require individuals who appear to the responsible person to be under 18 years of age (or such older age as may be specified in the policy) to produce on request, before being served alcohol, identification bearing their photograph, date of birth and a holographic mark.

Prevention of Crime and Disorder
3 The licence holder shall ensure that CCTV is installed at the premises and is maintained and in working order at all times. Images will be retained for a maximum period of 28 days and made available to any of the responsible authorities upon request.
4 The licence holder shall ensure that the premises have an industry standard approved intruder alarm system.

Protection of Children from Harm
5 The licence holder shall ensure that the ‘Challenge 25’ scheme is in operation at the premises.
6 The licence holder shall ensure that all staff will be trained in the sale of age restricted goods.
7 The licence holder shall ensure that there will be a system which prompts staff to ensure that they undertake age checks on restricted products.
8 The licence holder shall maintain a register of all challenged and refused sales.
