Annex 1 Mandatory Conditions
Mandatory conditions where licence authorises supply of alcohol
1 No supply of alcohol may be made under the premises licence:-
a at a time when there is no designated premises supervisor in respect of the premises licence,
or
b at a time when the designated premises supervisor does not hold a personal licence or his personal licence is suspended.
2 Every supply of alcohol under the premises licence must be made or authorised by a person who holds a personal licence.

Annex 3 Conditions attached after a Hearing of the Licensing Authority
Reduced Hours on Licence and Attached Conditions

Accepted proposed Conditions to be attached to licence.
The premises licence holder shall ensure all premises doors (with the exception of for entry or exit) and windows shall remain closed at all times from 22:30 hours onwards all nights of operation.
No alcohol (in any type of container) is permitted to be removed from the inside of the premises at any time from 22:30 hours onwards on all nights of operation.
The DPS of the Premises, or in their absence a senior member of the premises staff, shall make regular noise level surveys around the perimeter of the premises to ensure no noise is audible at the perimeters of the premises. The dates and times and name of the individual undertaking the survey, and the results of the surveys, shall be recorded in a bound book containing numbered pages. The book should be made available on request to any Enforcement officer working on behalf of the Local Authority or any Police Officer of South Yorkshire Police.
Polite signage to be displayed and maintained requesting patrons to leave the premises and the near vicinity in a quiet and responsible manner with consideration for the residential nature of the local area.
In addition the Designated Premises Supervisor is removed from this licence.
