Annex 1 Mandatory Conditions
Mandatory conditions where licence authorises supply of alcohol
1 No supply of alcohol may be made under the premises licence:-
a at a time when there is no designated premises supervisor in respect of the premises licence,
or
b at a time when the designated premises supervisor does not hold a personal licence or his personal licence is suspended.
2 Every supply of alcohol under the premises licence must be made or authorised by a person who holds a personal licence.
Mandatory conditions where a licence includes a condition requiring door supervisors at specified times (not applicable to Cinemas, Theatres or exclusively Club Premises)
3 All persons primarily engaged under or employed on the premises to vet, regulate and control persons entering the premises; or engaged, used or employed in the supervision of patrons once inside in order to ensure the maintenance of good order, public safety and internal security shall be licensed with the Security Industry Authority.

Prevention of Crime and Disorder
1. The licence holder shall ensure that CCTV is installed at the premises which meets South Yorkshire Police standards and images are retained for a period of at least 28 days.
2. The licence holder shall ensure that all members of staff receive training in and are aware of the need to ensure patrons do not become disorderly through excessive drink.
Prevention of Public Nuisance
3. The licence holder shall ensure that In order to ensure that the local neighbourhood is not disturbed by noise, the volume and control of any noise shall be such that it is not audible at the boundary of the nearest noise sensitive premises. Such control may include the adjustment of volume of both amplified music and public address systems, closure of doors and windows during an event, the use of noise control cut off devices and/or the introduction of sound insulation.
4. The licence holder shall ensure that the noise shall be monitored by the licence holder or his/her representatives to ensure compliance at reasonable frequencies during an event. The monitoring shall be logged and made available to the Licensing Authority at its request.
Public Safety

Protection of Children from Harm
5. The licence holder shall operate a strict identification procedure that ensures that young people do not gain access to the club or to alcohol without proof of age and identification
